Please watch the video clips and complete the readings associated with this module. Take what you learn from these materials and devise a short assessment tool. For example, create a survey or questionnaire, consisting of five relevant questions that can be used to evaluate a person’s entrepreneurial nature, skills, and/or mindset. Then take the survey (or questionnaire) you created and write a short summary of what you learned about yourself and your potential to be an innovator.
Use the following tips to develop a survey, if that is what you choose to do:
1. Please use a scale of 1 – 5. Here is an example.

2. Begin each survey question with “I” to make it personal. Here is an example:

4. Make sure the total score at the end of your survey reflects the extent to which you are innovative and/or entrepreneurial. Even if your overall score is low, please remember that skills can be developed over time.
